We’re excited to have you join our community. This form is required for all new members and serves as both your official membership sign-up and acknowledgment of Tan Peaks policies, including safety procedures, billing terms, and waiver of liability.

• This membership is a recurring monthly charge of $90.
• You will be billed monthly until you cancel in writing at least 3 days prior to your next billing date.
• Refunds are only issued in the case of billing errors, reported within 15 days.

This form must be completed and signed by the parent or legal guardian of any customer under the age of 18 prior to receiving any UV or red-light services at Tan Peaks. Completion of this form acknowledges that you have read and agree to the risks, policies, and procedures outlined herein. This form is valid for 1 year and will be kept on file at Tan Peaks.
