Completion of this form acknowledges that you have read and agree to the risks, policies, and procedures outlined herein. This form is valid for 1 year and will be kept on file at Tan Peaks.

This form allows you to temporarily freeze your Tan Peaks membership. All freezes must include a restart date and be approved by management.
Membership freezes are granted for up to 60 days, with access to all services suspended during this time. If you do not have a restart date or are unsure of your return, please complete a Membership C
